Dermal fillers are among the most commonly performed cosmetic injectable treatments. When appropriately selected and carefully performed, they have a well-established safety profile.
But dermal fillers are still medical procedures, and no injectable treatment is completely without risk.
At Nuvel Skin & Laser, we believe patients should understand both the benefits and the potential complications of filler treatment before deciding whether it is right for them.
The goal isn’t to make filler sound frightening. It’s to make sure patients can make an informed decision and understand why anatomy, technique, product selection, and preparation matter.
What Are the Common Side Effects of Dermal Fillers?
Most reactions after dermal filler treatment are temporary and occur around the injection site.
These can include:
- Swelling
- Bruising
- Tenderness
- Redness
- Temporary firmness or lumpiness
- Mild asymmetry while swelling settles
The amount of swelling and bruising varies considerably between patients and also depends on the area being treated.
For many treatments, patients can return to most normal activities quickly, although visible swelling or bruising can occasionally last longer.
What Complications Can Occur?
Less commonly, dermal fillers can cause problems that require assessment or treatment.
These may include:
- Persistent lumps or nodules
- Infection
- Delayed inflammatory reactions
- Unwanted product placement or contour irregularities
- Prolonged swelling
- Migration or an unnatural appearance from inappropriate product or placement
Hyaluronic acid fillers have an important advantage: if necessary, they can usually be dissolved using an enzyme called hyaluronidase.
However, there is another complication that deserves particular attention.
What Is a Vascular Occlusion?
A vascular occlusion occurs when filler interferes with blood flow through a blood vessel.
This is extremely uncommon, but it is one of the most important complications associated with dermal filler treatment.
If blood supply to an area of skin is compromised, the tissue can be injured. Prompt recognition and treatment are therefore extremely important.
Very rarely, filler entering the vascular circulation can affect the blood supply to the eye and cause visual loss. This is a medical emergency.
These complications are why filler should not be thought of simply as a beauty treatment.
A detailed understanding of facial anatomy and the ability to recognize and manage complications are fundamental parts of performing dermal filler injections.
Can Vascular Complications Be Completely Prevented?
Unfortunately, no technique can reduce the risk to zero.
Facial blood vessels vary between individuals, and even experienced injectors cannot assume that every patient’s anatomy follows exactly the same pattern.
Safety therefore involves several layers:
- Detailed knowledge of facial anatomy
- Appropriate injection technique
- Careful choice of injection plane and product
- Understanding higher-risk anatomical areas
- Recognizing warning signs quickly
- Having an emergency protocol and appropriate medications immediately available
- Knowing how to manage a complication if one occurs
The objective isn’t to pretend complications can’t happen.
It’s to reduce the likelihood of complications and be prepared to respond appropriately if they do.
Does Using a Cannula Make Filler Completely Safe?
No.
Cannulas can be useful in certain areas and may offer advantages for some injection techniques, but they do not eliminate vascular risk.
Similarly, needles aren’t inherently unsafe.
The safest approach depends on the anatomy being treated, the product being used, the injection plane, and the technique required to achieve the intended result.
There is no single device or injection technique that makes dermal filler risk-free.
What About Ultrasound?
Ultrasound is increasingly being used in aesthetic medicine to provide additional information beneath the skin.
At Nuvel Skin & Laser, ultrasound can be used when appropriate to help identify anatomical structures, assess previously placed filler, and assist in the evaluation and management of certain filler complications.
It is an additional tool—not a substitute for anatomical knowledge, careful technique, or clinical judgment.
We’ll explore the role of ultrasound in facial injectables in more detail in a separate article.
Why Your Medical History Matters
Safe filler treatment begins before the injection.
Your injector should know about relevant medical conditions, medications, allergies, previous cosmetic procedures, and any previous reactions to injectable treatments.
It’s also important to tell your provider about previous filler treatments, even if they were performed years ago or at another clinic.
Filler can sometimes remain in tissues considerably longer than patients expect, and previous treatment can influence future assessment and planning.
When Should You Contact Your Injector After Filler?
Most swelling, tenderness, and bruising after treatment are expected.
However, patients should contact their treating clinic promptly if something doesn’t seem right.
Particular concerns include:
- Significant or worsening pain
- Unusual changes in skin colour
- Skin that becomes pale, mottled, or dusky
- Rapidly increasing swelling
- Increasing redness, warmth, or other signs of infection
- Blistering or skin breakdown
Any change in vision following dermal filler treatment requires immediate emergency assessment.
When in doubt, we would rather hear from a patient than have them wait.
Safety Is More Than Avoiding Complications
Good filler treatment isn’t simply a procedure in which nothing goes wrong.
Safety also means avoiding unnecessary treatment, selecting an appropriate product, understanding when filler isn’t the right solution, and creating results that remain balanced with the rest of the face.
Sometimes the safest treatment decision is to use less filler.
Sometimes adequate structural correction requires more than a token amount.
And sometimes the right answer is not to inject at all.
